Getting Started

These materials and forms are supplied for clients to complete prior to their initial counseling session with me. Please print out the document, complete the questionnaires, provide the necessary signatures, and bring them with you to our first meeting.

Intake Materials Packet (.pdf)

This is a compilation of all Intake Materials necessary to begin the counseling process with me. It includes the following:

Information and Consent - Information regarding my professional background, confidentiality, length of sessions, and fees charged.
Notice of Privacy Practices - This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review this information carefully.
Insurance Information Form - Information required in order for me to bill your insurance carrier.
Intake Questionnaire - A series of questions designed to assist me in getting to know you better.

For Couples Only

In addition to the Intake Materials Packet, both members of the couple should complete this inventory separately (print, complete, and bring to counseling).
